2.Trend and Prediction of Urban Family Expenditure for Health Care

The increase of health care expenditures is an important problem in the almost countries. Also, suppression of the health care expenditures is an important problem in the health field of Korea since the national health insurance for total people in 1989. Thus, it is very important to grasp the change of the health care expenditures of family and proportions of the health care expenditures to total expenditures of family, because they are the basis of national health care expenditures in Korea. While the health care expenditures of urban family were increased during 1980-1993 by 12.8% annually, the total expenditures of urban family were increased by 14.8% annually. consequently, the proportions of health care expenditures to total expenditures were decreased from 5.98% to 4.76%. The proportions of health care expenditure for 3 years to come were predicted to 4.75% in 1994, 4.67% in 1995, and 4.63% in 1996 by the time-series ana]ysis. That is, it was predicted that they would be decreasing slowly. The product elasticity of health care expenditure was less than 1 in the multiple regression analysis, so the health care is normal good rather than superior good. Therefore, it seems that the household economy is able to bear the expense pursuing the improvement of quality of health care by actualizing the medical insurance fee.

4.A Case of Bullous Pemphigoid Associated with Graves' Disease.

We report herein a case of bullous pemphigoid(BP) associated with Graves' disease(GD) The patient was 43-year-old female who had suffered frorn GD for 8 years. She had annular, erythematous atrophic plaques with a margin of small vesicles on the frontal and temporal areas, Histopathologic findings of the skin lesion showed subepiderrnal bulla, and numerous eosinophilic infiltrate in the bulla cavity and dermis. Direct immunofluorescent study of perilesional skin revealed linear IgG deposition at the dermo epidermal junction. We think our case shows a higher than chance association of BP and GD because both are basecl on autoimmune mechanism.

6.A Case of Common Bile Duct Obstruction Associated with Duodenal Diverticulum.

The Duodenal diverticulum is the rare cause of the common bile duct obstruction. We experienced a case of the diverticulum of the second portion of duodenum in a 59-year-old male, who complained recurrent attacks of right upper abdominal pain and fever since 16 months prior to admission. Duodenoscopic finding and UGI study revealed a typical duodenal diverticulum just side to the papilla of Vater. Diverticulectomy and cholecystectomy was performed and after than, he has been free front above mentioned symptoms. We reported a case of duodenal diverticulum which caused recurrent common bile duct obstruction.

9.The Evaluation of SCC (squamous cell carcinoma antigen) Level as a Tumor Marker in Patient with Squamous Cell Carcinoma of the Cervix.

This study was performed to evaluate the clinical significance of SCC as a tumor marker in patient with squamous cell carcinoma of the cervix. We measured the serum levels of SCC by radioimmunoassay in patient with invasive squamous cell carcinoma of cervix to determine the prognostic value, correlation with the presence of lymph node metastasis, response to treatment, and those value in the early detection of recurrence after treatment. The result was: 1) In 117 of 174 patients with cervical cancer, the pretreatment positive rate of SCC was 47.0%(57/117). In each stage, the stage Ia was 25.0%, Ib 26.3%, IIa 56.0%, 62.6%, III 57.1%, IV 100%, and the recurrent case was 37.5%. 2) In 79 of 174 patients with cervical cancer, the pretreatment positive rate of SCC was 44,0%(30/69) in no evidence of malignancy patients, but 60.0%(6/10) in recurrent or permanent patients(p>0.05), 3) In 133 of 174 patients with cervical cancer, the posttreatment positive rate of SCC was 2.6%(3/117) in no evidence of malignancy patients, but 50.0%(8/16) in recurrent or permanent patients(p<0.05). 4) In 48 patients of 174 with cervical cancer who underwent radical hystrectomy, the positive rate of SCC was 71.4%(5/7) in pelvic lymph node positive patients but 19,5%(8/41) in pelvic lymph node negative patients(p<0.05). We concluded that the pretreatment SCC level was not effective as a prognostic value, but well correlated with pelvic lymph node metastasis, and serial measurements of serum levels of SCC provided a reliable clue for early detection of recurrence or progression of disease, so it may be useful for monitoring cervical carcinoma patient.

10.Analysis of Regular Oral Examination Results in 'A' Airliner Employees.

This study is based on the result of oral examination of 'A' airliner employees in the period of June 1997 and December 1997. The purpose of this study is to analyze the distribution of dental diseases among workers and the relationship between income level and dental healthiness. Based on the above analysis, this study further tries to indirectly evaluate the method of prophylaxis. Following is the list of basic findings of this study: Firstly, it was found that male workers tend to have inferior dental condition compared with female workers. Also, dental condition of technicians was found to be worse than that of office workers. Secondly, it was found that dental caries decreased with the age. However, periodontal disease was increasing with the age. Therefore, this result leads us to convince that the education should be focused on 'how to brush your teeth' in order to prevent periodontal diseases. Thirdly, more than 40% of the examined were found to be carrying dental calculus related diseases (67.2%) and cervical abrasion (40.5%), respectively. In conclusion, we became to know that the treatment of cervical abrasion and dental calculus at the Dental Clinic is effective in helping 'A' Airliner employees to have better dental status.
