1.Lymphomatoid Papulosis Presenting a Single Nodule.

Lymphomatoid papulosis is a chronic disease of cutaneous atypical lymphoid infiltration characterized clinically by involuting and recurring papules, plaques, and sometimes nodules. A 51-year-old man presented with a single, coin sized, nontender, erythematous nodule on the left thigh of a 2-week duration. There was a history of recurrence and spontaneous healing of similar asymptomatic eruptions five to six times over 20 years. A biopsy specimen showed a dense, wedge-shaped dermal infiltrate that was patchy and perivascular. The cellular infiltrate was polymorphous and consisted of large atypical cells, small lymphocytes, eosinophils and neutrophils. The large atypical cells were strongly CD30(Ki-1) positive. We report a case of lymphomatoid papulosis unusually presenting as a single nodular eruption, in which the differential diagnosis between lymphomatoid papulosis and CD30(Ki-1)positive large cell lymphoma is exceedingly difficult.

4.A Case of Calcinosis Cutis due to Intravenous Administration of Calcium Gluconate.

We report a case of calcinosis cutis following administration of 10% calcium gluconate, used for the treatment of a hypocalcemic seizure in a 5-week-old neonate. After administration of 10% calcium gluconate, subcutaneous induration developed at the infusion site. The histopathological findings taken from the dorsum of the left foot showed multiple foci of calcium deposits with infiltration of epitheloid histiocytes between collagen bundles and foreign body giant cells phagocyting calcific granules throughout the reticular dermis and subcutis.

6.A Case of Diffuse Biphasic Cutaneous Amyloidosis.

We described a case of diffuse biphasic cutaneous amyloidosis, a unique form of localized cutaneous amyloidosis. A 41-year-old man has gradually developed a lichenoid papular and a grouped spotted pigmented macular eruption on the trunk and upper extremities over the past 15 years. Histopathologic examination revealed that amyloid deposits were present in the papillary dermis. It was confirmed by Congo red staining, immunohistochemistry and electron microscopy. There was no evidence of systemic amyloidosis.

A 79-year-old man with balanitis circumscripta plasmacellularis(BCP), presenting as an erythematous constricting band of the inner surface of the prepuce encircling the penile shaft is described. The biopsy specimen of the lesion showed, in addition to the typical histologic findings of BCP, increased fibrosis and decreased amount of elastic fibers which correlate well with our clinical observations. Electron microscopic examination revelaed no viral particles or elastic fibers. Immunohistologically, IgG was found to be the major immunoglobulin class in the plasma cellular inf iltrate.

10.A Case of Extensive Classic Kaposi' s Sarcoma Showing Good Response to Radiotherapy.

We report a case of classic Kaposis sarcoma(KS) in a 74-year-old man who had asymptomatic red-dish-blue or dark-brown macules, papules and nodules on his bo hower extrimities. The biopsy specimen showed typical histopathologic features of FS including a highly vascularized lesion with a proliferation of spindle-shaped cells accompanied by etravasated erythrocytes and variable number of inflammatery cells. Many of the spindle-shaped cells associated with the proliferating vessels in KS lesion expressed factor VIII-related antigen. He was treated with 1500 cGy of radiation(300 cGy per day for 5 successive day. Four weeks after radiotherpy, gross and histopathologic findings were mucl improved and healed leaving hyperpigmentation.
